Initial Consultation – What to Bring

Prior to your first appointment, we recommend that you complete the new patient packet ahead of time. You can bring the completed packet with you to your appointment, or you may fax the packet to us prior to your appointment. This will save you the time of completing these forms at your appointment, and we will be able to serve you more quickly.

Items to Bring to Your First Appointment

  • Completed new patient packet
  • An insurance card
  • An ID card, such as a driver’s license
  • A list of all prescriptions and over-the-counter medications you currently take
  • A list of all questions and/or concerns that you may have about bariatric weight loss surgery

Insurance Frequently Asked Questions

We know when you are considering bariatric surgery, insurance coverage can be overwhelming. That’s why the MountainView Weight Loss Center staff will help you determine what services are covered by your insurance company. Fill out our insurance verification form or call us at (575) 521-8860 and we’ll review your insurance benefits and help answer any specific questions you may have about the requirements for coverage. Verifying benefits takes approximately 24 to 48 hours. If there are no benefits, we will assist you with other financing options.

Q: How do I find out if my insurance covers Bariatric Surgery?
Fill out our insurance verification form and we will submit this information to verify your benefits.

Q: How long does this process take?
It takes approximately 24 to 48 hours for us to determine if benefits are available. Once we verify your coverage, we will be able to tell you more about your insurance’s requirements for surgery, as the requirements vary by individual insurance policy. Our Insurance Coordinator will call you and review this information with you once it is obtained.

Q: If I have benefits available, how long will it take to get insurance approval and get surgery scheduled?
Once requirements are met and records are gathered, we will promptly submit the necessary medical documentation needed to obtain a letter of predetermination from your insurance company. The length of time this takes depends on your insurance company. In some cases, it can take up to six weeks.

Q: Can I do anything to speed the process along?
Yes! After your letter is received and put into the insurance company’s system, it is extremely helpful for you to call your insurance company on a regular basis to inquire about your letter of predetermination.

Q: What do I ask when I call my insurance company?
Tell the customer service representative that you are inquiring about the letter of predetermination documentation sent by Dr. McArthur’s office for bariatric surgery.

  • Ask them how long their predetermination process takes.
  • Call often! Our patients that call on a regular basis usually get their answer quicker than the patients that don’t call at all.

Q: How will I know if I am approved or denied?
Your insurance company will send you a letter by mail. You will usually receive your letter before our office does.

Q: If I am approved for weight loss surgery, what happens next?
Once we receive verification of approval from your insurance company, we will call you to schedule surgery.

Q: Do I have other options if my insurance does not cover or denies weight loss surgery?
Yes, we have financing options available. Call us, and we will guide you through this process if it is determined that your insurance will not cover weight loss surgery.

Support Groups: When & Where

Our support group is open to patients who are preparing for surgery as well as patients who have already had bariatric surgery. If you had surgery at another facility, please feel free to attend. The support group is coordinated by an RN who is a post-op bariatric surgery patient. This is a great place to learn from other patients, get your questions answered, and get some additional support throughout your journey. Everything discussed during the group will be kept confidential.
